A well-organized medical file helps a new physician see the full story quickly. Start with a one-page summary: your current diagnosis, major symptoms, previous treatments, operations, allergies and current medications.
Include pathology and imaging reports, recent blood tests and discharge summaries. If you have CT, MRI or PET imaging, keep the original DICOM files as well as the written report. Avoid compressing everything into screenshots when the original PDF is available.
